Requirements to obtain official transcript …

1)     Send $3.00 money order made out to August Martin HS for each official transcript. We DO
NOT accept personal checks.
2)     Print name as it was when you attended AMHS. If married, please give maiden name
3)    State whether discharged or graduated and what year.
4)   Date of birth
5)    Social Security Number
6)    Address where official transcript is to be sent
7)    Signature of student/guardian confirming permission for the release of record transcript.

PLEASE NOTE: When a student reaches 18 years of age, rights that were accorded to the
parent are transferred to the student. Therefore, parents/guardian no longer can obtain records
without written consent of the student.

Also, all book accounts must be settled before actioning your request.
